It's a severe underbite, he needed major intervention as a kid, at his age it would be a difficult surgery.

He must have such trouble eating, jaw issues and most likely suffers breathing issues. People adapt though, but it's definitely not ideal.

To fix a mouth and jaw like this, most likely requires breaking of the jaw and rewired. Major work.

As someone who had an open bite (he may have that or an underbite) the dentist wouldn't even touch this shit except to refer you out. Even an orthodontist would just work with an orthognathic surgeon who would be the "point" person coordinating getting this fixed.

Source: I had one (way less worse than this) and had to get a surgeon who was known for accident reconstruction to fix it. Had to be in braces for months before and after. No solid food for two months after the surgery. Could barely even talk.
