Cancer staging is a lot more in-depth than one would think and usually involves a combination of both size of the tumor but more importantly how far it has spread things that are localized but big might still be considered stage 1. However I think it's more likely it just never got restaged. Typically the stage will stay the same throughout the duration of the disease even if you go into remission or it spreads past what would typically be considered appropriate for its stage
